Your job in the department is to . . .

Work with internal and external partners to support and develop health and human service pathways and connections between the City of Snohomish and the community. In this role, you will spearhead the Department’s efforts to build collaborative relationships with community partners to champion the importance and value of equitable access to community-based services that respect each person’s unique experience. Your coordination, evaluation, and promotion of high-quality services will support historically disadvantaged communities in the City of Snohomish. Staff are expected to operate with considerable latitude and apply sound judgment and teamwork.

Building individual and community capacity in increasing knowledge and self-sufficiency through a range of activities such as outreach, community education, navigation, care coordination, social support, and advocacy. Coordinating initiatives to improve equitable access to social services, including City departments and community organizations as assigned. Your work will have a lasting impact on community-based services that respect each person's unique experiences, and the Department’s opportunities and strategies to promote diversity, equity, and inclusion.

Examples of Duties
Coordination & Cultural Mediation:

Focus on mental health, substance use, homelessness & housing, and connecting community members to services. Provides a conduit for organizations and providers to coordinate, collaborate and align resources in the City
Provide cultural mediation among individuals, communities, and health and social service systems (e.g. educate individuals and communities about how to use health and social service systems, build health literacy, cross-cultural communication)
Coordinate initiatives to improve equitable access to social services, including City departments; County, State and Federal agencies; and community organizations as assigned
Provide coaching and social support by motivating and encouraging people to establish care and utilize other community resources
Recruit and support youth and community members to participate in community-based prevention efforts, including behavioral health.
Develops, facilitates and supports community partnerships with school districts, colleges, first responders, substance use counselors, youth, housing, health and human service providers to reduce the prevalence of tobacco use, obesity and chronic disease. Assesses, promotes and supports the implementation of policy change where appropriate.
Focuses on disparate populations such as low-income, BIPOC, veteran, and Tribal populations, as appropriate
Program Management:

Propose policies, programs, and practices intended to improve equitable access to social services to the Director
Assess the benefit relative to cost of programs and policies intended to improve equitable access to social services
Interact and convene community partners and resources as necessary to assure population needs are being met
Education & Outreach:

Develop and deliver education materials to highlight available services for outreach efforts
Lead, convene, and participate in team meetings, training, and activities to further the health of the community
Reporting & Grants:

Develop and maintain record keeping systems and databases. Creates customized reports from various databases and applications using multiple delivery methods.
Responsible for grant writing, grant making, and grant oversight in support of relevant work.
Support:

Support advisory committees as assigned

Desirable Qualifications: Experience teaching, training, and working with individuals and organizations promoting equitable access to social services. Familiarity with historically disadvantaged groups. Fluency in English and one or more languages common to the historically disadvantaged. Extensive knowledge of the City and Snohomish County.

Supplemental Information
Job Conditions: This position primarily works some combination of hours Monday through Friday with the possibility of hours in the evening or on weekends. Driving conditions may vary due to extremes in weather. All staff positions at the City are required to be available to report to work during off hours in case of emergency and perform duties as assigned. Work may include traveling to community organizations such as schools, hospitals, clinics and social service agencies; other travel may include local or statewide for meetings and other forums.

Work Location: City of Snohomish

Travel: Within the City functions to perform functions such as, working with community members, presentations, and trainings. This can include outside Snohomish County.

The physical demands are typical of those that must be met by an employee to successfully perform the essential functions of this job. Environment: Tasks are regularly performed with exposure to adverse environmental conditions, such as dirt, pollen, odors, wetness, humidity, rain, fumes, electric currents, animals/wildlife, toxic/poisonous agents, violence, disease, or pathogenic substances. Tasks are routinely performed with exposure to temperature and noise extremes, machinery, vibrations, and traffic hazards.

Physical: Tasks involve the ability to exert moderate physical effort in light to moderate work, typically involving some combination of stooping, kneeling, crouching and crawling, and which may involve some lifting, carrying, pushing and/or pulling of objects and materials of moderate weight (20-50 pounds). Walk on uneven ground and negotiate stairs carrying equipment. Operate a SUV, mid-sized van, or similarly- sized vehicle, ability to address minor vehicle maintenance, and ability to physically assist individuals to enter, be seated, and exit the vehicle in a safe manner.

Vision: See in the normal visual range with or without correction; vision sufficient to read computer screens and printed documents; and to operate assigned equipment. Specific vision abilities required by this job include close vision, distance vision, and ability to adjust focus.

Hearing: Hear in the normal audio range with or without correction. Most tasks require visual perception and discrimination. Some tasks require oral communications ability. Some tasks require the ability to perceive and discriminate sounds.
